1. Please complete this form in its entirety, including the certification statement at the end of this document.
2. Press the "Submit Responses" button at the end of this form. The answers you supply on the form will be electronically submitted to the Advising staff for the Department of Anthropology.
3. This form is a department application only. There is a separate FSU online admissions application which is required by Florida State University.
4. When you have completed this application, you will be taken the the separate online admissions application.

You will also receive an automatic browser reply with your answers and an e-mail confirmation. It is recommended that you print out these responses and keep them for your personal records. If you did not receive a browser reply or e-mail confirmation, then contact the Anthropology Webmaster.
